Creativity flows in all ages. At Iris, we love to celebrate self-expression and create a safe space for everyone to explore it. We want to ensure a smooth and easy process for both you and your child. Here's everything you need to know when bringing a minor in for a piercing:
- Parent's ID/U.S. Driver's License/Passport
- Department of Health - Notarized Consent Form:
This form must be completed and notarized on the same day as the appointment.
Age Guidelines:
- Ear & Nostril Piercings: Available for ages 12+.
- Navel Piercings: Available for ages 15+.
- Other Body Piercings: (septum, eyebrow, mouth, etc.): Available for ages 16+.
Using the Online Notary Service:
If you choose to use an online notary, please follow these steps:
- Complete the Form: Fill out the consent form with the parent's information up until Item #5 (Location of the piercing), signing the document will be done with the notary
- Visit https://www.notarize.com/.: This website connects you to an online notary and takes about 25-45 minutes to complete. (not affiliated with us)
- Required Information: The parent will need their U.S. Driver's License, Social Security Number, and a stable WiFi connection.
- Note: The parent must use this link to create an account and complete the notary session. All information provided during the session must be for the parent only.
Please keep in mind, this service only accepts legal U.S. documents. If you have a foreign passport, you'll need to visit a physical notary.
Important Notes:
- We do not offer notary services in the studio due to legal conflict of interest.
- The notarized consent form must be brought with you on the day of your appointment.
- Other Body Piercings: (septum, eyebrow, mouth, etc.): Available for ages 16+
